A Letter from Our Community

As I reflect on my one-year anniversary at Hacienda Oaks Nursing & Rehab. I am thankful for the opportunity and have learned a great deal of knowledge as a Social Worker in Long Term Care. I must say my first year was faced with a few challenges which is expected with any new adventure. As a Social Worker at Hacienda Oaks I wear “many hats” which consist of completing assessments, setting up discharge plans, ensuring Residents and family members are satisfied with services (Grievance Coordinator), scheduling specialty physician in facility appointments, attending CARE plan meetings, serving as a member of the multidisciplinary team, keeping the Local Authority informed of Resident’s needs/concerns for residents whom receive PASRR services and much more. As you may have noticed I have a lot on my plate and I enjoy every minute of it. I am grateful to be part of a team that is dedicated to our Residents, family members, friends and community partners.
